Designing new models of care with health professionals


The client

The NHS Five Year Forward View was published in 2014 and sets out a vision for the future of the NHS. The document articulates why change is needed, what that change might look like and how it can be achieved.

To this end, in January 2015, the NHS invited organisations and partnerships to apply to become ‘vanguard’ sites for the new care models programme, supporting improvement and integration of services and the Five Year Forward View. Northumbrian Healthcare, in partnership with Northumberland CCG, led on a bid on behalf of the health economy in Northumberland to become an NHS Vanguard site in the category ‘integrated primary and acute care systems’. The successful bid has led to a funding boost over the next two years as local NHS and social care partners work collaboratively to pioneer new models of care shaped around patient needs.

The need

The Vanguard will transform the way healthcare is commissioned and provided across Northumberland with new models of care designed to transform outcomes for patients and ensure the public stay healthy and at their time of need have access to the best possible care at the right time and place.

To achieve this ambition and vision it is essential for GPs, Nurses, Consultants and other health care professionals to feel part of both setting the ambition and vision for this transformation and also identification of the challenges. As a result we were commissioned to engage with a range of health care professionals to understand the priorities and barriers for change.

The outcome

We engaged a wide range of health professionals including GPs, practice managers, practice nurses, hospital consultants, hospital nurses, community nurses, social workers and mental health professionals to capture their views via group sessions and one to one interviews.

The level of information collected was vast and included essential insights to help the Trust and CCG understand:

-         Where are we now in relation to integrated care

-         Where are the blockages in the system

-         What are the barriers for change

-         What are the priorities for change

This insight enables the Trust and CCG to develop models of care to address existing problems and build on existing best practice that is already present in Northumberland.
